Icy hot can freak kids out if you don't explain what it is and what's it's doing to them. All they know is that their skins is "burning", then "freezing". Little kids don't understand this sort of thing.

I'd assume this was part of their torture of this kid. Sort of a low-end psychological torture technique that works on kids
