It's because when you live in a rural area, you have to drive everywhere and you get to spend lots of time away from the eyes of strangers. If you live in an urban area, you can (often) do a lot of walking instead of driving and you get seen by hundreds of strangers everyday. So it's a combination of inherent lack of exercise and lack of shame.
