He had a botched back surgery. You can see the screws are in place but the bone is still overhanging by a lot.

Why didn't afro boy shoot the surgeon that probably got his degree from having the same tiny hat as the professor?

 Instead he shot Brian Thompson who was probably a figurehead fall-goy.
