What do all of these acts that are not only routine, but standard in porn, mean biologically?

Any way you look at it, it's plausible that what you said has some influence, but the chance is always much greater without contraceptives, even if the guy pulls out there's a chance of pregnancy and either way, accidents or "accidents" happen all the time, especially in the heat of the moment.
In porn the overwhelming majority ends up in a wrong ejaculation. It's not as important what you do during as what you do at the end. the pill has a failure rate. low but it's there and is also influenced by other medication.
condoms are near perfect if you use them right. They can break, sure, but as long as it's intact and stays on she can not get pregnant. Worst case scenario and you find it broke, you get the morning after pill. Not healthy but you will always know if a condom failed.

I never said the chance is not the highest without. It is definitely highest without.
I said that in the context of the motivation for sex. even if you use contraceptives, you ejaculate inside and TRY to get her pregnant.
