It IS horrifying. Don't forget they give you a paralytic but not pain meds. This snippet is probably being dramatic because the heart is the last thing to go so as if they wouldn't just take it and pretend they saw nothing, as he wouldn't be able to do much (or anything?). He probably woke up at the early stages of prep on the table, before cutting and paralytic.
