I asked myself that too, but given the context, I decided that I'm probably better off not knowing.
So I actually looked it up, from what I can tell it means "caregiver". I stopped there because when these degenerates claim they're something positive it typically means they're the opposite. The only caregiver in that freaks life should be the orderly snugging up the straightjacket and shoving chemical castration pills down that things throat.
